Swasthya Health Care provides diagnostic and pharmacy services including lab testing, home sample collection, and sale of medicines at discounted prices. Services are offered as per availability and scheduled appointments.
Booking and Appointments
Appointments for tests and home sample collection must be booked in advance through our franchise center, website, or mobile app. Swasthya Health Care reserves the right to reschedule appointments with notice.
Patient Responsibilities
Patients or their representatives must provide accurate personal and medical information necessary for testing and reporting. Cooperation with sample collection staff is mandatory for timely service.
Reports and Accuracy
Test results are generated by accredited labs under Swasthya Health Care's quality standards. While every effort is made to ensure accuracy, Swasthya Health Care is not liable for misinterpretations or errors due to external factors or patient misuse.
Privacy and Data Protection
Personal information and test reports are handled with strict confidentiality and in compliance with relevant laws. Data will not be shared with third parties without consent except as required by law.
Payment and Refunds
Payments must be made as per the applicable pricing at the time of booking or service delivery. Refunds or adjustments will be provided per the refund policy, applicable only for services not rendered or errors on the part of Swasthya Health Care.
Limitations and Liability
Swasthya Health Care is not responsible for delays caused by transportation, patient unavailability, or external lab processing. Liability for damages is limited to the service fee paid.
Cancellation Policy
Appointments can be canceled or rescheduled as per the cancellation policy stated during booking. Charges may apply for late cancellations or no-shows.
Use of Service
The services are intended for lawful and ethical use only. Any misuse, fraudulent activity, or violation of terms may result in service denial or legal action.
Changes to Terms
Swasthya Health Care reserves the right to update these Terms and Conditions periodically. Continued use of services after changes constitutes acceptance.
